PATIENT CASE STUDY 1

Patient A suffers from a respiratory disorder and requires regular check up appointments on her health. Her local hospital NHS Trust is not equipped or manned sufficiently therefore she is referred by her consultant/GP to a more specialised hospital unit.

She is assessed by her consultant/GP/authorising officer/nurse through a series of personal detail checks to have a requirement of hospital medical transport.

Once the hospital appointment has been confirmed the treating hospital transport department completes a transport booking form. This form then is emailed/faxed to CFY control and the details confirmed.

CFY control contacts the patient before the hospital date (where possible) to check/amend journey details and time of pick up. The transport booking is carried out and she receives her hospital treatment.

Her consultant at the treating hospital then decides on further appointments and transport bookings based upon her state of health. She then undertakes her return journey.

PATIENT CASE STUDY 2

Patient B suffered a minor heart condition whilst visiting relatives out of his locality. Once treated, he is referred back to his local hospital NHS Trust to continue rehabilitation.

He is assessed by his consultant/GP/authorising officer/nurse through a series of personal detail checks to have a requirement of hospital medical transport.

The transfer has been confirmed and there is a bed available locally and so the transport booking is made, completing the transport booking form.

This form then is emailed/faxed to CFY control and the details confirmed. CFY control contacts the patient before the hospital date (where possible) to check/amend journey details and time of pick up.

Patient B is discharged and transferred through us to his local hospital NHS Trust. Further transport bookings are made through us should there be any such requirement.
